Before continuing to the appropriate application, you will need the following documents:

IDENTIFICATION

• US citizens: One form of ID – Either State or Federally issued Identification card, such as Driver’s license, state ID or Passport

• Non US Citizen: Passport, Visa Page

PROOF OF INCOME OR GUARANTOR

• Two consecutive months of pay stubs, or three consecutive months of bank statements, or copy of last year’s tax return, or your W-9 or 1099, showing the total of income

• If you are a full or part time student and do not meet the minimum income requirements, you will need to have a Guarantor.

• Non US Citizen: I-20

$30 APPLICATION FEE

• We accept payments via credit card authorization form, US certified cashier’s check, traveler’s check or money order made payable to University Pointe.
